Hospital To Home (H2H)
Coming home has never been easier! The Hospital to Home service is catered specifically to older adults who are being discharged from a hospital or a skilled nursing facility. This service makes transportation and reacquainting with familiar surroundings easier and safer for seniors and their families following discharge. Transportation is performed by a registered, licensed and experienced caregiver.
NOTE: Hospital to Home does not provide medical transport or wheelchair accessible transport. Accommodations are limited to ambulatory and semi-ambulatory discharged patients.

Transportation from Hospital to Home
Picking up prescriptions*
Cleaning out and restocking refrigerator*
Meal preparation
Bathing
Preparing bedroom for first night’s sleep
*Prescription and grocery purchases are paid by client
4 hours $99
3 hr option available.
